Same as Charles posted— there is no in-store pickup available, so you can only use this offer if you have a bigger online order to be delivered.
It worked for me. I needed to choose a store a little further away than my regular one.

I assume you MUST use the app due to the code including ‘app’, but I didn’t try another method.
Sorry, unsure of expiration, but just ordered tonight, 10/30. Default order size was 20 cards so had to change that before submitting to 5.
